A narrative review compiled PubMed literature up to February 2026 on sodium–glucose cotransporter 2 (SGLT2) inhibitors in adults with prediabetes. The authors report that multiple studies show these agents can improve insulin sensitivity, produce modest weight loss, lower blood pressure and affect hepatic lipid metabolism. Two contemporary meta‑analyses included in the review found reduced incidence of new‑onset type 2 diabetes in adults with prediabetes who also have heart failure or chronic kidney disease, with relative risk reductions reported at about 19–21%. The review also summarises the established adverse signals such as increased genitourinary infections and rare euglycaemic diabetic ketoacidosis.

The review frames SGLT2 inhibitors as a promising option particularly for high‑risk subgroups with cardiovascular or renal disease, but it emphasises key limitations: the synthesis is narrative rather than a single pooled randomized‑trial meta‑analysis of prediabetes cohorts, and the authors call for dedicated randomized controlled trials in prediabetes with adequate follow‑up and washout periods before routine preventive use can be recommended. The paper notes that contemporary guidelines (to 2026) do not endorse SGLT2 inhibitors for prediabetes prevention outside existing heart‑failure and CKD indications.

Readers should interpret the review as an evidence synthesis that highlights potential signals and gaps rather than definitive preventive recommendations.
